ENTERING: Sweepstakes begins January 1st, 2020 at 12:01 AM CST and ends December 1st, 2020 at 11:59 PM CST. To enter, eligible

ELIGIBILITY: Open to U.S. residents age 25 or older. Entries must be complete to be eligible. This promotion is void where prohibited by law and where state licensing and/or registration requirements have not been met. Focus On Health employees and their immediate family members are not eligible to participate. Entries must be complete to be eligible. Limit one entry per person, regardless of entry method.

Approximate retail value per Grand Prize is $5000.00. Cash Prize is payable by services rendered. This Prize is non- transferable.

WINNING: Winner is guaranteed to be chosen by random drawing. One winner will be selected by random drawing from all eligible entries conducted by Sponsors on the second Saturday in January each year, at Sponsor's corporate address above, if delayed by weather the next work day sponsor is open. Winner need not be present to win. Official notice to winner will be sent by traceable method of delivery, return receipt requested. The selected prize must be claimed within thirty (30) days of notification. If the prize is not claimed, it will be awarded in a subsequent random drawing. If prize is unclaimed by second winner it will remain unrewarded. Winner is responsible for any income tax or tax withholding's resulting from prize. Maintenance fees and assessments or back maintenance fees in arrears are the responsibility of the winner. To receive the prize, winner must sign an Affidavit of Authorization and Release, including consent to use of name, photo, likeness and city and state of residence by Sponsors for the purpose of advertising, publicity and promotion without additional compensation or permission unless prohibited by law.
ODDS OF WINNING: The odds of winning the prize depend upon the number of eligible entries received, or 1 in 1000.
